Clyde A. Erwin Middle

We will be hosting a physical clinic after school on Thursday, May 16th.  The cost of the physical is $15 (cash or check only) and the money goes back to Erwin Middle Athletics.  This is for any student currently in grades 6-11 and will cover them for the entire 24-25 school year.  Parents need to plan on picking up their child beginning at 4:15 at the Erwin High School Gym.  In order to receive a physical, a parent or guardian must fill out and sign pages 1 & 2 of the physical packet.  Your child needs to bring this packet with them on the day of the event.  Packets can be found in the main office or by clicking this link.
